Exciting News for Conklin

We’re thrilled to introduce our new Community Support Worker, Shawna Tremblay, who is dedicated to strengthening our Conklin community through working to connect individuals to partnerships, resources, and culturally grounded support. Here’s a snapshot of her new role:

  • Building Connections: Forge partnerships with residents, families, and local agencies to promote healthy lifestyles, identify possible issues, and provide crisis intervention by linking people to essential services.
  • Personalized Guidance: Offer one-on-one case management, navigation, and follow-up to support individuals and families in recovery, personal development, and well-being.
  • Cultural Focus: Collaborate with agencies and Indigenous Elders to weave our traditions into programs, fostering cultural pride, and solutions that are good cultural fit.
  • Clear Communication: Serve as a bridge between our community and external organizations – sharing info, attending workshops, and ensuring quick resource connections.
  • Trust & Engagement: Cultivate strong relationships through regular check-in, positive outreach,  and fun community events to boost involvement and awareness.
  • Resource Hub: Maintain a network of partners and create an easy-to-use community guide with key contacts and services.
